Referrals from professionals regarding the care of an adult

If you're a GP, health worker or professional representing an external organisation, you can make an online referral on behalf of an adult.

How to ask for an assessment

You can ask for an assessment from adult social care if you may have need for care and support.

You can complete a self-assessment, or request an assessment from one of our social work teams.

If you need help with this, someone else can ask for this on your behalf (such as a friend, family member or professional).
