Referrals from professionals regarding the care of an adult

If you're a GP, health worker or professional representing an external organisation, you can make a referral on behalf of an adult online.

How to ask for an assessment

You can ask for an assessment from adult social care where it appears that you may have need for care and support.

You can access tailored information online, complete a self-assessment, or request an assessment from one of our social work teams.

If you need help with this, someone else can ask for this on your behalf (such as a friend, family member or professional).
