Community care centres

Community care centres provide a wide range of services to support people in the community.

The centres support:

adults living with dementia and other conditions, and their carers
adults to be discharged from hospital for a period of assessment and reablement
local people - the centre will be a hub for the local community with integrated services tailored to the needs of the community they serve

The centres have open public areas, centre-based services to meet assessed needs, and provide a base for community support home care staff, as well as short and long term residential care.

Open public areas

The aim of the public areas is to provide an open access resource for older people in the community and their carers. Anyone can access these services and there is no need to have a formal assessment.

Services based on individual support plans

Services which may be available following a full assessment of need include:

intensive day care places available 7 days per week
day opportunities in a range of multi-purpose rooms and facilities which can provide assessment, therapy, treatment and social activities
an assessment service for people with dementia, and carer peer support groups
promotion of carer's assessments and carer's emergency card
arts projects and therapy
memory box groups, carers and cared for people helped to develop memory boxes

Specialist home care is available to complement the centres and includes:

a team dedicated to support local people with dementia, within a specialist service specification
a flexible service supporting carers in crisis

Residential accommodation and support

Within each centre there are between 32 and 40 residential bedrooms with en-suite facilities.

They also include a range of computerised electronic systems throughout the building that will, for instance, reduce the risk of falling by allowing lights to come on automatically when residents get out of bed in the night or detect when residents may have unexpectedly left their rooms.

Long-term residential care

CQC registered long-stay beds in units of up to 8 people based on individual person-centred plans
self contained and safe
strengthening existing skills and focus on empowering individuals
providing opportunities for activities
specialist support, within the Derbyshire dementia care pathway

Short-stay residential accommodation

There are Care Quality Commission registered places providing short breaks and intermediate care.
