Apply for a Gold Card

If this is your first application for a Gold Card you can apply in one of several ways.


Online

If you are applying for your first Gold Card because of age you can complete the application form and upload your photograph online. Checks will be carried out to verify your age. You cannot apply for a Gold Card online if you are applying because of a disability. You will need to contact your local district, borough or Derby City Council.

At a free photography session

Download the application form attached to this page, or pick one up from your local district, borough or Derby City Council or any Derbyshire library.

Take the completed application form and any original proof documents to one of the offices listed on the application form. Check if you need to make an appointment first.

In person with a photograph

Download the application form attached to this page, or pick one up from your local district, borough or Derby City Council or any Derbyshire library.

Visit your local council office with a completed application form, a recent, colour, standard passport-sized photograph with your name and address written on the back and original proof documents, not copies. Accepted proof documents are listed in the application form.
