New financial assessment (or change of circumstances)
If you are someone who has been asked to complete a financial assessment because you have recently started to receive adult social care support in the community, please continue to complete the financial assessment and ensure you press the submit button as this information is required in order to complete your financial assessment.
Public consultation – request for revised charges under the proposals
If you are someone who is already receiving adult social care support in the community and pay a co-funding contribution and want to know as part of the public consultation how each of the 3 options would affect you, please continue to complete the financial assessment.
Please be aware, when you first start you will be asked to tick a series of consent and declaration boxes but once you have entered your information you will be provided with the estimated figures and don’t need to press the submit button. If you do not press the submit button, then none of your information will be passed to us.
