Joined Up Care Derbyshire brings together 12 organisations across the county and sets out the ambitions and priorities for the future of Derbyshire's health and social care services.
The partnership has developed a sustainability and transformation plan focussing on:
- helping to keep people healthy
- giving people the best quality care
- running services well
- making the most of available budgets
This sets out a vision for a more joined up approach to health and social care, the steps that need to be taken to get there and how people should be involved.
