Joined Up Care Derbyshire
Health and social care organisations in Derbyshire have been working together closely for some time to improve services for people and increase efficiency and effectiveness.
Joined up Care Derbyshire is the name of the programme which bring together 12 organisations across the county and sets out the ambitions and priorities for the future of Derbyshire's health and social care services. The partnership has developed a sustainability and transformation plan which focused on:
- helping to keep people healthy
- giving people the best quality care
- running services well
- making the most of available budgets
This sets out a vision for a more joined up approach to health and social care, the steps that need to be taken to get there and how people should be involved.
