What this consultation is about

The Derbyshire Better Care Fund

People across Derbyshire were invited to have their say on a new fund called the Better Care Fund. The Better Care Fund supports initiatives that will see health and social care services working more closely together.

We are joining with the county's five clinical commissioning groups (CCGs) - Erewash, Hardwick, Southern Derbyshire, North Derbyshire and Tameside and Glossop through a new agreement, under Section 75 of the National Health Service Act 2006. This will start in April 2015.

The aim is to build on work carried out already across the county to ensure health and social care services work in a more integrated way, benefiting residents, improving services and helping organisations to work more efficiently and effectively.

The Derbyshire Better Care Fund has agreed a pooled budget of £61.489m. The plan details a number of initiatives and aims including:

  • reducing admissions to hospitals and residential care homes
  • increasing prevention services which help people stay living safely and well at home for longer
  • strengthening communities and community health and social care services available for vulnerable people
  • more proactive services offering support and help at the earliest opportunity
  • ensuring more support and services are available at weekends
  • giving more support to carers and implementing the Care Act.

All of the partners to this agreement have consulted on their integrated health and care plans over the last two years, for example the CCG five year plans. However, we would also like to hear your views on the new, combined agreement.
