How to apply for, renew or replace a Blue Badge

Blue Badges can be applied for on an individual basis or on behalf of an organisation.


Apply for a new Blue Badge

You can apply for a Blue Badge for yourself, or on behalf of a child.

The applications forms are available by:

  • downloading an application form attached to this page
  • completing the online request form (a form will be posted out to you).

Filling in the application

You should write on the application form.

Enclose two passport size photographs. If you can't get this type of photo, use two current snapshots of a similar size clearly showing your face.

Enclose photocopies of the evidence document to support your application. If you send the original it won't be returned to you.

Post your completed form, photos and documentation to:

Paying for your Blue Badge

It costs £10 for each successful application. You can pay for your blue badge online, by telephone, cheque or postal order.

Lost or stolen badges

If your badge has been lost or stolen then please complete the the form attached to this page. Post it to:

Renewing a Blue Badge

If you already have a Blue Badge you will automatically receive a renewal notice. If you've changed your address since your current badge was issued please Call Derbyshire.
