Our aim is to support people to live as independently as possible, and offer choice and control in the way that is achieved. Some of the ways we can assist is through: Support with housing needs, both long-term and short-term: - This may be signposting and supporting people with rehousing to meet long term needs. - Assessing and supporting people with adapting their home with both smaller adaptations and major adaptations. This may include a disabled facilities grant application, subject to meeting the relevant legislation. - Supporting people to return to living within the community from residential facilities. Assessing for specialist equipment Assessments for activities of daily living Assessments for safely moving and handling, to assist both clients and carers (family or paid) Reablement services may be supported by OT goal setting if necessary We work closely with health occupational therapy services who may also support with some of the above (especially learning disability and community therapy teams).
