Continuing Care is free care outside of hospital that is arranged and funded by the NHS. The care can be provided in any setting, including residential schools, residential placements, hospice or the home of the person you look after. The service complete assessments against the adult criteria for continuing healthcare for young people with severe or complex needs. On-going case management and review for eligible young people.
