How to get a Gold Card
Step One
First of all you need an application form. Use the links from this page to go to the information for the area in which you live. The application process does differ slightly across the county so it is important to check the information for your local area.
If you prefer you can pick up a Gold Card application form from your local council office, library or participating post office.

Step Two
Complete the application form and make sure you have the documents shown on the form to prove that you are entitled to a card.
Step Three – APPLY!
You can then choose one of three easy ways to apply:
Apply in person at your local council office – follow the links on the left to find the location and opening times for the office closest to you.
Applying at a Post Office – the links on the left to your area will list the Post Offices at which you can apply (please note you cannot apply at Post Offices in Chesterfield, Derby or Erewash).
Apply by post – the links on the left will provide information about where to send your application if you would like to apply by post.
