Workplace health and wellbeing staff survey

A workplace health and wellbeing staff survey is a simple way to collect anonymous information about the health and wellbeing needs of the employees in a workplace.

It takes approximately 15 minutes to complete online.

For the purposes of anonymity, the Health and Wellbeing Staff Survey is available for workplaces with 10 or more employees only.

The survey asks a range of health and lifestyle questions including general health and wellbeing, smoking, healthy eating, alcohol, physical activity, sleep, menopause, work and health, workplace culture, workplace illness, health and wellbeing support and a section for line managers.

Once completed a report will be provided to the workplace summarising the results and possible recommendations to address any identified health and wellbeing needs.
