Aftercare service

What sort of things can I expect from the Aftercare Service?

The Aftercare team in your local area will provide a range of services including:

  • Social activities to help you meet other young people who are leaving or who have already left care.
  • Help to find accommodation when the time is right to leave care.
  • Help with cooking and the practical things you need to know.
  • Help with filling in forms.
  • Help putting you in contact with organisations like Connexions and specialist services.
  • Help coping with the many unexpected things that can pop up when you live on your own.
  • Provide you with your own copy of our leaving care guide "Getting Sussed".

What does your Aftercare Personal Advisor do?

Your Aftercare Personal Advisor will do all of the above and is also there to make sure that you always have someone who you can contact to sort out any problems when you leave care.  They will keep contact with you until you reach 21.

How much contact you have depends on how much you need but as a minimum the Aftercare Personal Advisor will be in touch at least four times a year.  When you first leave care though, you could be seeing each other several times a week!

How much will it cost?

As a care leaver you are entitled to leaving care services free of charge.

Will we pass on information about you to other organisations, such as Connexions and housing organisations?

We will only pass on basic information such as name and date of birth, unless you tell us that you do not wish us to do so.  Your Personal Advisor will discuss which organisation might need to be involved and will seek your views on sharing information.

What if I have more questions?

If you have more questions, you can contact the Aftercare Service in your area or your social worker.

Who do I need to contact?

You don't really need to contact anyone because our systems have been designed to ensure we contact you before your reach 16.

You can find out more by reading the Leaving and aftercare procedures document in the more information section.
