Services for disabled children
We have two social work teams for severely disabled children and young people.
One mainly based in Chesterfield, covering the north of the county and one based in Heanor, covering the south of the county.
These teams provide a full range of social work roles, including:
- carrying out needs led assessments
- working with children, families and other agencies to provide advice and support
- promoting social inclusion.
Examples of help can be supporting disabled children to access local community based activities or services, or, for some children, attending specialist services such as outreach or short-term break provision.
The service takes into account both the needs of the children and their carers during the assessment process.
