Care plans and reviews

If you are eligible for day care, your Key Worker (a member of staff who provides individual support) will write a care plan with you and your carer (where appropriate), that will clearly set out all the services the assessment indicates you need.

All parties will then agree how these services should be delivered and staff will work from the agreed care plan. 

The care plan will be reviewed with you and anyone else involved in your care, on a regular basis to ensure your care needs continue to be met. 

When day care is no longer appropriate, staff will assist you where possible, in accessing other services.
