Social care & health
Anyone may become disabled, suddenly through accident, or gradually through illness, and some people are disabled from birth.
Society is not designed for disabled people. Our aim is to help you get over the problems this causes. We use a set procedure in assessing your needs.
This document is called Meeting Assessment Responsibilities Under the NHS and Community Care (NHSCC) Act 1990. The document can be viewed or downloaded from the more information section on this page.
Some of the following may be familiar to you.
Has your everyday life become more difficult?
For some people a simple aid may make all the difference. Other people may need alterations to houses or flats. We can advise on this. You may be able to get a grant towards the cost.
Do you need help with household chores, or personal assistance?
We may be able to provide a home help or home care aide who can do jobs such as housework, lighting fires in the morning, or helping you to get up. If you have young children, help may be available with their daily care.
Do you need to learn new ways of doing things?
Our occupational therapists may be able to help you find easier ways of coping with domestic tasks in your own home. Our staff can help you think about employment and social activity. Transport may be available to centres.
Leaving school soon?
We can help you think about your future. If you need our services we can make the necessary plans.
Carers have needs too.
We will do all we can to help. This may be a break for a few hours, or a week, or perhaps putting you in touch with other carers.
Who can you talk to?
Our area offices can give help and information about local groups.
Please use the following link to find out how to contact your local area office.
