Blue badge scheme – How to apply

You can request an application form by ringing Call Derbyshire on 08456 058058 or, you can complete the online request form below and click the submit button to send it to us. We will then send you an application pack by post.

This form should only be used to request an application pack for a new blue badge. If you need to renew your badge, a renewal form will be sent to you before expiry of your current badge.
