Services for adults

We support you in maintaining or, if necessary, regaining the skills to live as full and independent a life as possible.

If you are experiencing domestic abuse please ring the domestic abuse helpline on 08000 198668.

You will receive an assessment of your needs. This is based on the eligibility framework for social care services, which is in line with Government guidance. We also discuss your needs with other agencies, such as health, where appropriate.

A care plan will be written with you and your carer (where appropriate) if you are eligible for assistance.

This care plan will clearly set out all the services the assessment shows that you need. All parties will then agree how these services should be delivered and staff will work from the agreed care plan.     

The care plan will be reviewed at regular intervals with you and your carer (if appropriate) and adjusted as necessary.

How to contact a social worker

If you need to contact social work staff for advice or an appointment, all the telephone numbers and email addresses are available in our Social Care Contacts section. You can use this section to contact the area office closest to you, or if you have an emergency outside normal office hours, the contact details for the out of hours service are also available.
