Community Care Centres
Community Care Centres provide a wide range of services to support people in the community.
The centres support:
adults living with dementia and their carers
local people - the centre will be a hub for the local community with intergrated services tailored to the needs of the community they serve.
The centres have open public areas, centre based services to meet assessed needs, are the base for community support home care staff as well as short and long term residential care. The building and services have been designed so that all public, specialist therapeutic and private lining areas are linked and work in harmony.
Open public areas
The aim of the open public areas is to provide an open access community resource for older people and their carers. Anyone can access these services and there is no need to have a formal assessment.
Together with reception facilities we provide:
access to information and advice including Derbyshire Information Portal, Alzheimer's Society, Age Concern Derbyshire, Derbyshire library service, bookings for professional social care, referrals via Call Derbyshire
cafe / bistro - also the venue for dementia cafes
internet cafe / shop / hairdresser
private rooms - for confidential matters
Telecare demonstration suite
fitness / meeting rooms (book-able for local community groups)
adult education groups
peer support groups
health promotion events and fire service home safety assessments
signposting to other services with a non-dementia focus.
Services based on individual support plans
Services which may be available following a full assessment of need include:
15 - 20 intensive day care places available seven days per week - six days will be in place by end of April 2011
day opportunities in a range of multi-purpose rooms and facilities which can provide opportunities for assessment, therapy, treatment and social activities
an assessment service for people with dementia, and carer peer support groups
promotion of carer's assessments and carer's emergency card
arts projects and therapy
memory box groups, carers and cared for people helped to develop memory boxes.
Specialist home care is available to complement the centres and includes: - proposed
a team dedicated to support local people with dementia, within a specialist service specification
a flexible service supporting carers in crisis.
Residential accommodation and support
Within the centres there are:
32 residential bedrooms with en-suite facilities.
a range of computerised electronic systems throughout the building that will for instance, reduce the risk of falling, by allowing lights to come on automatically when residents get out of bed in the night; detect when residents may have unexpectedly left their rooms.
Long term residential care
16 CQC registered long stay beds in units of up to eight people based on individual person centred plans
self contained and safe
strengthening existing skills and focus on empowering individuals
providing opportunities for activities
specialist support, within the Derbyshire dementia care pathway.
Short stay residential accommodation
There are 16 Care Quality Commission registered places providing short breaks and intermediate care. These are split as follows:
Short breaks
eight short stay beds and communal areas: four respite, four long term beds at present - respite beds will increase as long term beds become available on the first floor
integration with other services within the centre and beyond
planned and emergency short breaks.
Intermediate care - five beds at present - three reablement
eight short stay beds and communal areas providing respite and short breaks providing
a mixture of social care support and therapeutic input for a short period (usually around six weeks) intended to increase an individual's physical, emotional and mental functioning
facilitating discharge form acute settings, and to support people to return home or to prevent hospital admission or long term care.
Where are Community Care Centres in Derbyshire?
Staveley Community Care Centre is our flagship site which is based in Middlecroft, Staveley, Chesterfield. We are currently developing a second centre which is combined with an Extra Care housing scheme in Swadlincote. It is anticipated that this will be ready in June 2012. We are looking at developing other community care centres across Derbyshire.
